How Long Will My Botox/Neurotoxin Results Last?

Patients ask me this question all the time, and my answer is always the same: it depends.
Most people can expect their Botox results to last about 3 to 4 months. That said, I have patients who are ready for treatment again right at 12 weeks, and I have others who can easily go 5 to 6 months between appointments.
I personally only get treated every 5 to 6 months.
The reason there's such a wide range comes down to factors like your metabolism, activity level, dosage, and even what you consider "wearing off."
What Does "Wearing Off" Actually Mean?
This is where a lot of confusion happens.
Many patients think Botox is either working or not working. In reality, it's a gradual process.
Most people will start noticing movement returning before the product has completely worn off. That's normal.
When patients tell me, "I can move my forehead again," my first question is usually, "Okay, but how much?"
As an injector, I don't define results by whether your face is completely frozen. I look at things like:
How much movement has returned
Whether your wrinkles are still softened
Whether we're preventing deeper lines from forming
Whether your static lines are improving over time
My goal isn't to make you expressionless. My goal is to reduce repetitive muscle movement that causes lines to become etched into the skin.
That's what gives you the best long-term results.
Why Some People Need Treatment Every 12 Weeks
After treating patients for the last three years, I've definitely noticed certain patterns.
One of my patients is on testosterone therapy and works out five days a week. Like clockwork, he's ready for treatment again at 12 weeks.
I have another patient on a GLP-1 medication who also tends to need treatment closer to the 12-week mark.
On the other hand, I have patients who can comfortably go 5 or even 6 months between treatments and still look great.
Everybody metabolizes neuromodulators differently. That's why I don't love one-size-fits-all treatment schedules.
The Dose Matters More Than People Think
This is probably one of the biggest reasons people feel like their Botox didn't last.
Patients are often nervous about looking frozen. I get it. Nobody wants to look overdone.
I tell patients all the time that I would rather start conservatively than make someone uncomfortable with their results.
Sometimes I'll recommend 40 units and the patient will immediately say, "That sounds like a lot."
My response is usually something like, "That's okay. We can start with 30 to 35 units and see how you do."
But I also tell them the same thing every time:
There's a good chance it won't last as long, and next time you'll probably want the full dose.
Most of the time, that's exactly what happens.
Less product can absolutely be the right choice for some people. Patients should always feel comfortable with their treatment plan. They just need to understand that reducing the dose often means reducing the duration as well.
Can Aftercare Affect Results?
I believe aftercare matters.
I recommend avoiding strenuous exercise for 24 hours after treatment.
Could a single workout completely ruin your Botox? Probably not.
But if you're investing in treatment, it makes sense to follow the aftercare instructions your injector gives you. I want patients to give themselves the best chance of getting optimal results.
"My Botox Only Lasted Two Weeks"
I've heard this more times than I can count.
I've had patients come to me after being treated elsewhere and tell me their Botox only lasted two weeks.
When we start talking through it, that's usually not what actually happened.
Many times they got a good result, started noticing movement return, and assumed everything was gone.
Other times they were underdosed.
Sometimes I pull up before-and-after photos and show them that while movement has returned, they still have significantly less movement than they did before treatment and their lines are still much softer.
Those conversations are a good reminder that movement returning does not automatically mean the treatment has stopped working.
My Philosophy on Botox
I would rather slightly undertreat than overtreat.
That's probably not what most people expect an injector to say.
My goal has never been to make someone look frozen. I want people to still look like themselves.
I spend a lot of time evaluating movement patterns, muscle strength, facial balance, and the tiny details that many people overlook.
That attention to detail comes from years working in the ICU, serving in the Army, becoming a Family Nurse Practitioner, and now spending the last several years focused on aesthetic medicine.
I've always been obsessed with the details.
The best compliment my patients can receive isn't, "Wow, your Botox looks amazing."
It's when someone says:
"You look really good lately. What have you been doing?"
That's the goal.
How Often Should You Get Botox?
I don't believe everyone needs to be on the exact same schedule.
For most first-time patients, I typically recommend rebooking around four months.
If they feel like they need treatment sooner, we can always adjust moving forward.
What I'm really looking for is consistency over time.
The goal isn't to chase a perfectly frozen forehead. The goal is to prevent lines from becoming deeper and more permanent year after year.
The Bottom Line
Most Botox results last about 3 to 4 months.
Some people will be ready for treatment again at 12 weeks. Others can go 5 to 6 months.
The biggest factors I see in my practice are metabolism, exercise habits, GLP-1 use, dosage, and consistency with treatment.
If you're wondering how long your results will last, the honest answer is that nobody can predict it exactly.
What I can tell you is that a personalized treatment plan will always give you better results than a cookie-cutter approach.
Because the goal isn't to look different.
The goal is to still look like you, just a little more refreshed.
